5 weeks annual leave after two years
Sick leave entitlement of 5 days will be available from commencement of employment and another 5 days at 6 months of employment. 10 days entitlement will be available at 12 months service thereafter.
About the role:
Health Coaches are based in general practice and support people by taking positive steps towards improving their health. This includes:
Empowering patients to take control of their health and wellbeing
Encouraging good management of various health issues that patients want addressed
Discuss how to be more active by doing things they enjoy
As you will also have the flexibility of working in the community, you will have a significant impact on people's well-being and ability to make progress. The key word is “support”, walk alongside people, supporting them to achieve their goals. This can include:
Providing support to people and their whānau in their own community
Connecting to services in the community
Working with people towards their health and wellbeing goals.
To be successful in this role you will:
Be willing to embrace new ways of working
Have a helpful, flexible, positive, and adaptable disposition
Thrive in a busy and client-focused team
Be able to work at a fast pace and maintain a flexible schedule
Have a history of working with diverse patient groups of all ages and stages
You will be well supported in this role, initially receiving training through Health Literacy alongside a comprehensive 3-month induction to help you fully integrate the role into the practice/s.
We strongly value your growth and development as a Health Coach/Community Support Worker, and you will receive additional professional development to support you in this new role.
As this role will be supporting small, rural practices, travel between sites will be required.
